This page explains what information this website collects, and what we do with it. We kept it in plain English on purpose. If anything is unclear, call us at (773) 989-2500 and we'll explain.
The short version
- Browsing this site doesn't require you to give us anything.
- If you book an appointment or use the chat, we collect what you type so we can help you.
- We use your information to schedule visits, answer questions, and provide care. We do not sell it.
- Your medical records are protected by federal law (HIPAA) and are handled separately from this website.
What we collect, and when
Just browsing: nothing personal. Our hosting provider keeps standard server logs (things like your IP address and which pages were requested) to keep the site running and secure.
Booking an appointment: the scheduler on our booking page asks for your name, phone number, email, and the reason for your visit. That information goes to our practice management system so we can confirm your appointment and get in touch.
Using the chat: anything you type in the chat bubble, plus the contact details you provide, goes to our office so we can respond. Please keep detailed medical questions for your visit or a phone call. Chat is best for scheduling and quick questions.
Calling or emailing us: we keep what you share with us so we can follow up.
Tools this site uses
A few parts of this site are run by companies we work with:
- Scheduling and chat run on LeadConnector, our practice's patient communication platform. It processes what you submit and may set its own cookies.
- Videos are hosted on Vimeo. A video only connects to Vimeo when you press play, and Vimeo may set its own cookies at that point.
- Hosting is provided by Vercel, which keeps the standard server logs mentioned above.
The site itself does not run advertising trackers, and we do not sell or rent your information to anyone.
How we use your information
- To schedule, confirm, and remind you about appointments.
- To answer the questions you send us.
- To provide your care once you become a patient.
If you give us your phone number or email, we may use it to contact you about your appointments. If you ever want us to stop, just tell us.
Your medical records
Once you're a patient, your health information is protected under HIPAA, the federal health privacy law. How we protect it is described in our Notice of Privacy Practices, which you'll receive at the office and can request any time. This website policy covers the website; the Notice of Privacy Practices covers your medical records.
Children
This website is not directed at children. If you're a parent booking care for your child, you provide the information on their behalf.
Changes to this policy
If we change how the website handles information, we'll update this page and the effective date at the top.
